Trespass Enforcement Request Form
There are two ways to submit a Trespass Enforcement Request form --
print and
mail OR online. In
either case, renewal notices are sent by e-mail. Please make sure to
include your e-mail address on the form in the space provided.
Please take a moment to review the
instructions
for completing the form. If the form is not completed correctly,
it will be returned to you.
If you choose to use the online form, please be aware of the
following:
- You should have version 9 or later of the free
Adobe Reader
for the online form to work properly.
- Fill out the required fields on the form then select the
"E-mail Form" button. The form should attach itself to an e-mail
using your e-mail provider. Send as you would a normal
e-mail. We have found that some providers automatically send
the form without opening your e-mail.
If you are having trouble getting the form to send,
try saving it to your computer and then attach it to an e-mail
as you would any other attachment. Send to:
police.info@mesaaz.gov.
- At the bottom of the form where you would normally sign
your name, simply type your name in the field. This should
create a "digital signature" if you have one set up
already or ask you if you want to create one.
By typing
your name in this field, you are, in effect, signing the
Trespass Enforcement Request and agreeing to the
stipulations of the program as outlined on the form.
You do not need to physically sign the form if you
are submitting it electronically. However, it
MUST have a digital
signature. If you send the form
electronically, please DO NOT mail the form as well.
- If you are completing the form online, do not
rename the form or send as anything other than its
original form (PDF file). You may save the form to
your hard drive to fill out later and then attach to an
e-mail as the original PDF file, and send to:
police.info@mesaaz.gov. For example, do not
print the form, fill it out, scan it, save it as a JPG or
other graphic file and then send that attached to an e-mail.
- Because this is a court
document, we MUST have the original letter with your
physical signature OR the PDF online form with your digital
signature. We cannot accept scanned or faxed copies.
- You will not receive an automated response when
you e-mail the form. You will received a response from
us when we receive your form in our e-mail box. (Regular
business hours apply for response.) If
you do not receive a response from us that we've received
your form within a few business days, you may try submitting again or print the
form and mail it in.
