Accessing the Claims and Health Information Portal (CHIP)


An online tool that allows employees to view their health insurance benefits and claims (bills) anytime. 

Reset Password at 460-644-2299Information Available on CHIP:

  • Covered Members on your Plan(s) under age 18

  • Your spouse and dependents over age 18 will have their own user name and password to their health information

  • Plan Benefit Summaries

  • Deductible Information

  • Claim (bill) History

  • Benefit-Related Links

  • Current Life Insurance Beneficaries and option to change Beneficiaries


Member Login Info

Insured members of the City of Mesa may login to CHIP using their Insured ID and password which is provided by the Benefits Office. This login information ensures your privacy, so it should not be shared.

 After three failed attempts at logon, the system will lock you out for 10 minutes and then you may retry. 


If you have forgotten your password and need it reset, call 480-644-2299, Monday - Thursday, 7 a.m. - 6 p.m. You will need to answer security questions in order to have your password reset. Also, for your security, passwords are not sent via email. Return to Member Login


Viewing Health/Dental Claim (Bill) Status

  • Click on Member Login or Provider Login (Physicians). 
  • Input your Insured ID and password, and then read the website terms.
  • Click "I accept" if you agree to the terms of use of the website.
  • At the "Member Information" screen click "select" next to the name of the member whose claims you'd like to view.
  • From the navigation tree on the left click "Claim Status" and a list of claims processed will appear.
  • Select the desired claim.
