An online tool that allows employees to view their health insurance benefits and claims (bills) anytime.

Member Login Info
Insured members of the City of Mesa may login to CHIP using their Insured ID and password
provided by the Benefits Office. This login information ensures your privacy, so it should not

Viewing Health/Dental Claim (Bill) Status
- Click on Member Login or Provider Login (Physicians).
- Input your Insured ID and password, and then read the website terms.
- At the "Member Information" screen
click "select" next to the name of the member whose claims you'd like to view.
