Information Portal (CHIP)
CHIP is
An online tool that allows employees to view their health insurance benefits and claims (bills) anytime.
Information Available on CHIP
Member Login Info
Insured members of the City of Mesa may login to CHIP using their Insured ID and password
which is
provided by the Benefits Office. This login information ensures your privacy, so it should not
be shared. After three failed attempts at logon the system will lock you out for 10 minutes and then
you may retry. Passwords expire every 60 days-so if you haven't logged in
lately, give Benefits a call to reset your password at 644-2299
between 7am-6pm Mon-Thurs. For your security, passwords are not sent via email.

Viewing Health/Dental Claim (Bill) Status
Click on Member Login or Provider Login (Physicians). Input your Insured ID and password, and then read the website terms. Click
"I accept" if you agree to the terms of use of the website. At the "Member Information" screen
click "select" next to the name of the member whose claims you'd like to view. From the navigation
tree on the left click "Claim Status" and a list of claims processed will appear. Select the desired
claim.

access to health-related and personal information to only those
people who need to know it in order to provide customer service
and support.
