Plan Document describes the benefits available to eligible plan
participants. The Plan described in this document is effective, January 1,
2011 except for those provisions that specifically indicate other effective
dates. If you are not eligible for certain coverages, then the chapters
pertaining to those coverages do not apply to you.
This document will help you understand benefits provided by the City of
Mesa. As the Plan is amended those amendments will be posted below.
