If you are a new athlete or have an expired Arizona Special Olympics Physical, print the Medical Release Form. It must be filled out by a Physician. To verify that you have a current physical on file please contact Jacquie Gallo or call her at 480-644-4948.

Inclusion Services

The City of Mesa provides additional staff and support for all recreation programs that allow individuals with disabilities to mainstream into their regular park district programs. Staff and volunteers will be partnered with individuals who require special assistance.
