Calls for Service Request Form

Use this form to request a grid summary or calls for service for a specific apartment/condo/townhome community. A reply to your request will come from the following e-mail address -- karen.hubbard@mesaaz.gov. If you have spam blocking software or are restricting e-mail to only people in your address book or pre-approved e-mail addresses, please add this address so our reply will not be blocked or considered spam.

Please make sure your e-mail address is correct before submitting this form. If your e-mail is returned to us as undeliverable for any reason (including being waitlisted), your request will not be processed any further.

Any request for information other than what was outlined on the previous page will be required to submit a Public Records Request.

SCHOOLS:  If you are requesting calls for service for school property OR information pursuant to the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act, do not use this form. You must submit a Public Records Request.

LAW FIRMS:  Attorney's must submit a Public Records Request.

Verification of address will be done prior to running request. We will not run specific addresses for single family residential homes, duplex/triplex or fourplex units. We will not run specific apartment unit numbers.
