By completing this application, I agree that the City of Mesa is not responsible for replacing banners that are stolen, damaged, or destroyed due to age, vandalism, or any acts of nature including high winds.  Any loss or damage to an installed banner will be handled at the discretion of the City.  Number of banners and location is subject to change.  Banners will be maintained with collaboration from the Downtown Mesa Association.

Once contacted for required forms and photo, I hereby grant the City of Mesa permission to use the provided photo (which includes the likeness of myself, my relative, or my sponsored veteran or service member) in the Hometown Heroes Banner program without payment or other considerations.

Once form is submitted you will be contacted by a City of Mesa representative to complete the application process.  You may be asked to provide:

Proof of Residency
Verification of Active Duty Status
High Resolution 5" x 7" Photo or Digital Photo
Receipt From Sponsor Donation
