Physically Challenged information is now available through MASD.

If you are a new athlete or have an expired Arizona Special Olympics Physical, print the Medical Release Form. It must be filled out by a Physician. To verify that you have a current physical on file please contact Jacquie Gallo call her at 480-644-4948.

Inclusion Services

The City of Mesa provides additional staff and support for all recreation programs that allow individuals with disabilities to mainstream into their regular park district programs. Staff and volunteers will be partnered with individuals who require special assistance. Partners will facilitate participation and inclusion in recreation programs allowing for an optimal leisure experience.

Registration dates and fees vary according to the program interested in joining. You can register using our online system or in person at the Parks & Recreation S. Center office.

Please note: a medical physical form and waiver is required to participate in all sports programs.
