Instructions for Persons with Disabilities and
Persons with Limited English
If you require special assistance such as:
- I am disabled or elderly and cannot complete the online application
- I need an application in Braille or large print
- I speak another language like Chinese or another language and cannot read English, Vietnamese or Spanish. Paper applications have been translated into these languages. For other languages, translation services are available.

You will be asked to complete:
- The “Request for Reasonable Accommodation Request” form. This can be completed at any time before or after 10:00 a.m. December 6, 2016 and before 5:00 p.m. December 19, 2016. You can return it by fax, email, or in person at the City of Mesa Housing Authority office during business hours: Monday-Thursday 7:00 a.m. to 6:00 p.m.
- Once your request is approved, you will be given the assistance requested. If it is a request for a paper application because of a disability, you will be given until January 3, 2017 @ 5:00 p.m., Arizona Standard Time to complete the application.
You may also go to one of the following locations listed below, on December 6th only for on-site staff assistance. Library computers include ADA computers.

If you need transportation, please contact Valley Metro for assistance 602) 253-5000 or TTY (602) 251-2039. Persons with disabilities can request transportation through East Valley Dial A Ride at (480) 633-0101.
Online applications are in English, Vietnamese and Spanish. Paper applications are available in Spanish, Vietnamese and Chinese. All translations services are available during business hours at the Mesa Housing Authority Office from 10:00 a.m. December 6, 2016 and before 5:00 p.m. December 19, 2016.
