An affidavit of Change/Correction can be used when three (3) or fewer changes to a recorded Final Plat or Minor Plat are proposed. These changes may include lot line adjustments, lot combinations, bearing or distance changes, or other minor corrections such as language of dedication, notes, or legal description.
The main steps in the Affidavit of Change/Correction process are:

Request a Pre-Submittal Conference online.

Prepare and submit all necessary documents for formal submittal online.

Staff Review
Receive review comments and revise and resubmit documents online.

Recording
Submit the final document for recording.
A Pre-Submittal application is required prior to filing an application for an affidavit of Change/Correction. Pre-Submittal applications are due each Monday by 12:00 pm. and must be submitted via the DIMES portal.
Staff will review the Pre-Submittal application for compliance with all appropriate standards, codes, specifications, and requirements and provide written review comments within 8 Mesa working days.
Applicants will have the option to attend a Pre-Submittal Conference to discuss the provided comments. Pre-Submittal Conferences are typically a half hour and held each Tuesday morning. Conferences may be requested to be held in person or electronically.
Please note that a Pre-Submittal does not constitute a complete review or guarantee that all issues are identified.
Required Documents:
- Application - submitted through the DIMES portal
- Project Narrative – describing, in detail, the proposed changes.
- Sketch Plan – showing proposed changes to the Final Plat or Minor Plat.
Desired Documents:
- Title Report – along with all recorded documents showing proof of ownership.
- Copy of the recorded Final Plat or Minor Plat.
- Affidavit of Change/Correction consisting of the following:
- Written description of the corrections and/or changes proposed
- The Docket and page number of the recorded plat (Final Plat or Minor Plat) which is to be modified, and the date the plat was recorded.
- Exhibit A, consisting of the following:
- A depiction of how the plat is recorded with all affected sections of the plat identified, titled As Recorded.
- Exhibit B, consisting of the following:
- A depiction of the changes and/or corrections proposed with all affected sections of the plat identified, titled As Amended.
Complete applications and fees must be submitted electronically via Mesa’s DIMES portal. Applications are due each Monday by the end of the business day.
The applicant is responsible for the accuracy and completeness of all information submitted. Incomplete applications will NOT be accepted.
Required Documents:

- Property Owner Authorization(PDF, 162KB)
- Written consent of the change or correction by the current owners of record of all of the property affected by the change or correction, as shown by a title report furnished by the owner and prepared within 30 days prior to the filing of the Change or Correction.
- Title Report – along with all recorded documents showing proof of ownership.
- Copy of the recorded Final Plat or Minor Plat.
- Project Narrative – describing, in detail, the proposed changes.
- Affidavit of Change/Correction consisting of the following:
- Written description of the corrections and/or changes proposed
- The Docket and page number of the recorded plat (Final Plat or Minor Plat) which is to be modified, and the date the plat was recorded.
- Certification by the registered land surveyor preparing the document that the information provided is correct and accurate, and that the monuments described in it have either been set or located as described. All pages shall contain the seal of a registered land surveyor, as per Arizona Revised Statutes (ARS).
- Exhibit A, consisting of the following:
- A depiction of how the plat is recorded with all affected sections of the plat identified, titled As Recorded.
- Exhibit B, consisting of the following:
- A depiction of the changes and/or corrections proposed with all affected sections of the plat identified, titled As Amended.
Staff will review the Affidavit of Change/Correction for compliance with all appropriate standards, codes, specifications, and requirements and provide written comments within 8 working days of the application acceptance.
This comment letter will identify requirements that need to be resolved before obtaining approval.
The applicant will revise pertinent documents to address review comments and provide a written comment response letter stating how each review comment was addressed.
This process repeats until all requirements are met.
Once all applicable standards, codes, specifications, and requirements have been met, the applicant will prepare the Affidavit of Change/Correction for approval.
The applicant is responsible for recording the Affidavit of Change/Correction with the Maricopa County Recorder’s office. Once recorded, the applicant is to upload a copy of the recorded document to the case file in the DIMES system and notify the assigned Planner that it has been uploaded.
