Mesa offers a host of health and wellness benefits from medical, dental and vision coverage, to flexible spending accounts and income protection plans such as short-term disability and life insurance. The New Hire Benefits Guide below will provide you with an overview of the benefits available with plan highlights, eligibility, instructions on how to enroll and monthly costs.

Be prepared to make your benefit elections

The following is a checklist to help you keep track of what you will need to do prior to your New Employee Orientation (NEO) meeting:

1. Read and familiarize yourself with the New Hire Benefits Guide prior to the meeting. Call the Employee Benefits office with any questions you may have, otherwise, be prepared to ask your questions at the NEO meeting.
2. Locate and bring photocopies of any applicable verification documents if you are intending to enroll your spouse/committed-partner and or eligible dependents to any benefit plan coverage (e.g. marriage certificates, birth certificates, adoption/legal-guardianship paperwork, Affidavit/Declaration of Committed Partnership(PDF, 235KB), etc.) See Dependent Eligibility Section of the Guide for additional information.
3. Bring social security numbers (don't bring a hard copy of cards) for all your eligible dependents you are intending to enroll.
4. You may need to complete a Medical Health Statement (MHS)(PDF, 204KB) form for certain coverage amounts above the guaranteed issue levels for Voluntary Life Insurance on yourself or eligible spouse (spouse's signature required if electing coverage above guaranteed issue). Enroll using eBenMesa

• During your NEO meeting, you will be provided with detailed information about how to enroll using eBenMesa, our online enrollment tool; you will also be asked to update your contact information via an online ESS application.
• Be prepared to make your beneficiary designations via eBenMesa during your NEO meeting (can also be updated throughout the year).
  • Note that Arizona is a community property state. If you are designating any portion of the employee Basic or Voluntary life insurance to someone other than an applicable spouse as primary beneficiary, you must have your spouse complete a spouse waiver(this does not apply to committed partnerships). Life insurance for a spouse and/or dependent children are automatically designated to the employee as primary beneficiary and cannot be changed.
