Application materials can be obtained by mailing a written request to:

All materials MUST be submitted via Zoom Grants. 

Resolution of Support Required Format and Information***

NOTE: Each request for resolution of support to apply must be made separately and include the name of the tribe to which you will be applying.

Your formal request for a resolution of support must be made in letter format on agency letterhead, addressed to Mayor Mark Freeman, and MUST include the following information:

1. The name of the program/project your agency/department will fund with the grant.
2. The amount of funding requested from Ak-Chin Indian Community.
3. A short description of the program/project for which you are requesting funding.
4. A brief paragraph describing how this program/project will impact Mesa community AND the projected number or percentage of Mesa residents to be served.
5. Your complete contact information including phone, email and mailing address with a designated point of contact.

NOTE: Include your COMPLETED and SIGNED Ak-Chin application cover sheet with your emailed materials.

• The Applicant section should be all applicant information. Do not include any city information in that section.
• The date at the bottom where the mayor signs must be left blank for him to write in the correct date when he signs.
• For the Fiscal Agent Typed/Printed Name and Title line, include Mark Freeman, Mayor, City of Mesa

All requests for City Council Resolutions of Support must comply with the above and be received via ZoomGrants on or before March 23, 2026.

NOTE: Your agency must be located in and/or serving the residents of Mesa in order to be eligible for City of Mesa Council support.
